I. POSITION SUMMARY: Describe the major purpose or objective of the position. The Hospitalist RN (HRN) is a member of the hospitalist team who is responsible for delivery of Sound Core Processes designed to improve quality and efficiency in the care of hospitalized patients. The HRN works closely with the Chief Hospitalist to engage and educate the physician team, ensure performance of Sound Core Processes and support the hospitalist practice. This position does not involve providing direct patient care.
Sound's Core Processes - Performance Delivery 1) Participate in development of tools and workflows under direction of Regional Nurse Manager (RNM) 2) Work with the Chief to implement Sound Core Processes under the direction of the Regional Team and the Clinical Operations Team 3) Support and adhere to defined processes and participate in optimization 4) Provide timely status updates and respond to requests in a timely manner 5) Understand, clearly articulate and promote adoption of Sound's Core Processes to physician team and hospital departments 6) Collect and report program metrics (e.g., communication/query tools placed, recommendations accepted/declined) 7) Attend required meetings on time and as scheduled: o Regular meetings with Regional Nurse Manager and Regional Chief Medical Officer o Quarterly Performance Reviews and Quarterly Business Reviews with Chief and Regional team o Hospitalist team meetings o Hospitalist RN educational sessions and workgroups o Coordinate and facilitate Efficiency Forum meetings o Other meetings as defined by the Chief, RNM, and CMO 8) Engage and Educate Hospitalist Team: o Daily communication with hospitalist team to identify patients approaching discharge, to address pending queries and missing quality measures, etc. o Work with Chief to educate physicians on Sound's Core Processes and other opportunities to improve team or hospital performance o Assist Chief and CMO in providing performance data to the physician team to drive continuous improvement o Support Regional Team in presentation of Sound's Core Processes results during Quarterly Business Reviews. 9) Identify opportunities for improvement in tools and workflows
* Current RN licensure in good standing, in the state applicable to this position * Strong clinical knowledge and experience in a Critical Care setting (Critical Care setting = ICU, CCU, or Emergency Dept experience). Emergency Department experience requires other inpatient nursing experience. * Knowledge of process improvement methodology * Ability to assess patient's clinical status and readiness for discharge * Ability to work independently * Ability to work collaboratively * Excellent organizational skills with the ability to multi-task and prioritize workload in a fast-paced environment * Excellent written and oral communication skills * Ability to interact and communicate with individuals at all levels (e.g., patients, physicians, hospital staff, hospital administration and other colleagues) * Strong interpersonal skills to handle sensitive situations and confidential information. Position continually requires demonstrated poise, tact, diplomacy, and good judgment. * Strong presentation skills * Attention to detail and accuracy * Client-service oriented * Proficiency with technology, including electronic medical systems and Microsoft Office Suite
Education Minimum: RN Preferred: BSN Experience Minimum: 3 - 5 years inpatient nursing experience, with at least 2 years in a critical care setting, including exposure to electronic medical systems. Preferred: 5 or more years inpatient nursing experience in a critical care setting, experience using electronic medical systems.
